feat(16-02): create shared types package

- @codewalk-district/shared exports AppRouter and entity types
- Re-exports Initiative, Phase, Plan, Task, Agent, Message from schema
- Types-only package with no runtime dependencies
- TypeScript resolves directly (no build step needed)
