In YOLO mode, if mergePhaseIntoInitiative() threw (e.g., branch
doesn't exist, merge conflict), the error propagated and completePhase()
was never called — leaving the phase permanently stuck at in_progress.
Also wrap per-phase recovery in try-catch so one failing phase doesn't
abort the entire recoverDispatchQueues() loop.
When an agent stops, check whether a quality review should run before
auto-completing the task. If shouldRunQualityReview returns run:true,
delegate to runQualityReview (which transitions task to quality_review
and spawns a review agent) instead of calling completeTask directly.
Falls back to completeTask when agentRepository or agentManager are
not injected, or when the task lacks phaseId/initiativeId context.
- Add agentManager optional param to ExecutionOrchestrator constructor
- Extract tryQualityReview() private method to compute branch names and
repo path before delegating to the quality-review service
- Pass agentManager to ExecutionOrchestrator in container.ts
- Add orchestrator integration tests for the agent:stopped quality hook

Three fixes for phases getting stuck when a detail task crashes and is retried:
1. detailPhase mutation (architect.ts): clean up orphaned pending/in_progress
detail tasks before creating new ones, preventing duplicates at the source
2. orchestrator recovery: detect and complete stale duplicate planning tasks
(same category+phase, one completed, one pending)
3. ensureBranch: catch "already exists" TOCTOU race instead of blocking phase
- Remove original task blocking in handleConflict (task is already completed by handleAgentStopped)
- Return created conflict task from handleConflict so orchestrator can queue it for dispatch
- Add dedup check to prevent duplicate resolution tasks on crash retries
- Queue conflict resolution task via dispatchManager in mergeTaskIntoPhase
- Add recovery for erroneously blocked tasks in recoverDispatchQueues
- Update tests and docs
Adds DiffCache<T> module, extends BranchManager with getHeadCommitHash,
and wires phase-level caching into getPhaseReviewDiff and getFileDiff.
Cache is invalidated in ExecutionOrchestrator after each task merges into
the phase branch, ensuring stale diffs are never served after new commits.

When merge_and_push failed at the push step, the local defaultBranch ref
was left pointing at the merge commit. This made the three-dot diff
(defaultBranch...initiativeBranch) return empty because main already
contained all changes — causing the review tab to show "no changes."
Now mergeBranch returns the previous ref, and approveInitiative restores
it on push failure. Also repaired the corrupted clone state.
Three bugs causing empty phase diffs when server restarts during agent
execution:
1. Startup ordering race: reconcileAfterRestart() emitted agent:stopped
before orchestrator registered listeners — events lost. Moved
reconciliation to after orchestrator.start().
2. Stuck in_progress tasks: recoverDispatchQueues() only re-queued
pending tasks. Added recovery for in_progress tasks whose agents
are dead (not running/waiting_for_input).
3. Branch force-reset destroys work: git branch -f wiped commits when
a second agent was dispatched for the same task. Now checks if the
branch has commits beyond baseBranch before resetting.
Also adds:
- agent:crashed handler with auto-retry (MAX_TASK_RETRIES=3)
- retryCount column on tasks table + migration
- retryCount reset on manual retryBlockedTask()
approveInitiative was merging and pushing with a stale local
defaultBranch, causing "rejected (fetch first)" when origin/main
had advanced since the last project sync. Now fetches remote and
fast-forwards the target branch before merging.
handleTaskCompleted and handlePhaseAllTasksDone both bailed early when
initiative had no branch, silently skipping phase status transitions.
Also, merge failures would skip the phase completion check entirely.
- Decouple phase completion check from branch existence
- Wrap merge in try/catch so phase check runs even if merge fails
- Route updateTaskStatus through dispatchManager.completeTask when
completing, so the task:completed event fires for orchestration
Critical: review/merge tasks hit an early return in handleTaskCompleted()
that skipped the phase completion check, leaving phases stuck in
in_progress forever. Changed to an if-block wrapping only the merge step.
Also adds requestChangesOnInitiative() which creates/reuses a
"Finalization" phase for initiative-level review feedback, with dedup
guards for both phase and initiative request-changes flows.
Completed phases showed "No phases pending review" because:
1. Frontend filtered only pending_review phases
2. Server rejected non-pending_review phases
3. After merge, three-dot diff returned empty (merge base moved)
Fix: store pre-merge merge base hash on phase, use it to reconstruct
diffs for completed phases. Frontend now shows both pending_review and
completed phases with read-only mode (Merged badge) for completed ones.
Both phaseQueue and taskQueue are in-memory Maps lost on restart. Now
the orchestrator's start() method scans active initiatives and:
- Re-queues approved phases into the phase dispatch queue
- Re-queues pending tasks for in_progress phases into the task dispatch queue
- Triggers a dispatch cycle if anything was recovered
This fixes stuck phases/tasks after server restarts.
The in-memory phaseQueue (Map) in DefaultPhaseDispatchManager is lost on
server restart. After approving a phase review, dispatchNextPhase() found
nothing in the empty queue, so the next unblocked phase never started.
Now the orchestrator re-queues all approved phases for the initiative from
the DB before attempting to dispatch, making the queue self-healing.
When all phases complete, the initiative now transitions to
pending_review status instead of silently stopping. The user
reviews the full initiative diff and chooses:
- Push Branch: push cw/<name> to remote for PR workflows
- Merge & Push: merge into default branch and push

- Wire frontend: mutation replaces stub handler, window.prompt for
optional summary, loading state on button
The orchestrator's agent:stopped listener only called scheduleDispatch(),
never marking the agent's assigned task as completed. Tasks stayed
in_progress forever after their agent finished.
Now handleAgentStopped() calls dispatchManager.completeTask() before
scheduling new dispatches, respecting requiresApproval and skipping
manual (user_requested) stops.
ExecutionOrchestrator now listens for phase:queued and agent:stopped
events to drive the dispatch cycle, closing the gap between queueing
phases (Execute button) and actually spawning agents. Coalesced
scheduling prevents reentrancy with synchronous EventEmitter.
Move src/ → apps/server/ and packages/web/ → apps/web/ to adopt
standard monorepo conventions (apps/ for runnable apps, packages/
for reusable libraries). Update all config files, shared package
imports, test fixtures, and documentation to reflect new paths.
Key fixes:
- Update workspace config to ["apps/*", "packages/*"]
- Update tsconfig.json rootDir/include for apps/server/
- Add apps/web/** to vitest exclude list
- Update drizzle.config.ts schema path
- Fix ensure-schema.ts migration path detection (3 levels up in dev,
2 levels up in dist)
- Fix tests/integration/cli-server.test.ts import paths
- Update packages/shared imports to apps/server/ paths
- Update all docs/ files with new paths
